1 pound ground beef
1 onion, finely chopped
1 green bell pepper, finely chopped
8 ounces mushrooms, finely chopped
2 cloves garlic, minced
1 cup breadcrumbs
1/4 cup milk
1/4 cup ketchup
2 tablespoons Worcestershire sauce
2 teaspoons Italian seasoning
Salt and pepper to taste
1 cup shredded provolone cheese
1 cup shredded mozzarella cheese
Sliced provolone cheese, for topping
Sliced green onions, for garnish


Instructions:

Preheat your oven to 375°F (190°C). Grease a 9×5-inch loaf pan and set aside.
In a large skillet, heat some olive oil over medium heat. Add the chopped onion, bell pepper, mushrooms, and garlic, and sauté until softened.
In a large mixing bowl, combine the ground beef, sautéed vegetables, breadcrumbs, milk, ketchup, Worcestershire sauce, Italian seasoning, salt, and pepper. Mix until well combined.
